Does it have to be down to the bare aluminum or if the paint is in decent shape could it just be applied over it to form the seal?

For the best results it needs to be totally clean of any paint and other substances. That way it can seep into the cracks and crevices and seal all the leaks. If paint is in the way the seal will not be as good and may fail down the road.
