Bought an 08 Tohatsu 20 hp not long ago. Exact same symptoms. My other motor is a mid 80's Jonson 15 hp. It's stream from the water pump is not all that vigorous. But I was suspicious of the Tohatsu. I tore it down and found the impeller was both cracked and worn. I replaced it and found the the pee stream started very quickly at idle and was much stronger than before.

As for degree of difficulty--I'd give this job about a 3 on the 1-10 scale. The biggest issue on my motor was that in order to disconnect the shift linkage I had to remove a tiny pin by tapping it out with a tiny long straight object of a slightly smaller diameter. Found a drill bit that worked and the rest was pretty straight forward.

I found a U Tube video that helped.

If in doubt replace the impeller, much easier than fixing a overheated top end!

Was it running well and then stopped flowing a lot of water, or is it new to you so you don’t know the history?

I just replaced the impeller in a 3.5 Johnson, and it flows well even at idle.
